The vehicle manufacturer lists conditions that may require adherence to the ..severe maintenance.. schedule. If vehicle usage or climate conditions meet any of the ..severe.. criteria you are required to adhere to the severe maintenance schedule as listed by the vehicle manufacturer. 2. Acceptable records documenting the purchaser..s adherence to the vehicle manufacturers.. required maintenance must be provided in the event of a claim. 3. Acceptable records must be: issued on the date of maintenance, be issued in the name of the purchaser, be signed by the purchaser, be electronically generated, be issued by the commercial lubrication service facility performing the maintenance, include mileage, date, vehicle identification number, year, make and model, and include a record of payment and service performed. Any handwritten information on receipts, records or repair orders, pertaining to, or documenting required maintenance will not be accepted. Exclusions: 1. Failure of any listed component prior to benefit activation as stated above will not be covered by this program. 2. Failure of any component when the vehicle..s odometer reading has exceeded 250,000 miles or 10 years have elapsed from the sale date of this program will not be covered. 3. Any and all claims or damage resulting from: abuse, negligence, freezing, overheating of any type, failure to maintain proper coolant and fluid or lubrication levels, personal damages, per diem expenses, storage fees, medical expenses, telephone or rental charges, fire, flood, vandalism, theft, collision, acts of God, racing, usages not approved by the vehicle manufacturer, improper load capacity, or improper towing, commercial usages, damages caused by contaminated fluids or lubricants for any reason, misuse, road conditions, alterations not approved by the vehicle manufacturer, riots, or acts of war, diesel engines manufactured prior to 1990, rotary engines, excessive oil consumption and diminished performance is not covered. Parts or components that have been damaged by excessive heat or scoring are not covered. Electrical components are not covered by this program. 4. Pre-existing conditions and problems that occur prior to the benefit activation date and mileage are not covered. Parts that are worn or burned but NOT broken are not covered. Broken parts are defined as parts that have cracked or separated into pieces. 5. Any component or part not listed as a covered part will not be covered. Incidental and consequential damage is not covered by this treatment program. 6. Any vehicle with a non-functioning odometer is not eligible for this program. Claims occurring outside the United States are not covered. 7. Any work or repair done to your vehicle without prior written authorization from the administrator will not be considered a valid claim. 8. Limit of Liability: per single claim is $4,000 or the actual cash value of the vehicle, (wholesale value) as determined by the Kelly Wholesale Blue Book or the NADA Wholesale Guide whichever is the lesser amount at claim time.
